• Certificate of Good Moral Character (Elem./H.S Principal)
• Birth Certificate
• Dental Cetificate (For SSL application only)
• Health Certificate (For SSL application only)
• 1"X1" ID Picture
• Honorable Dimissal

2. Old Student

• Clearance
• Student Report Card
